EAS Academy Graduate Scholarship Application Form


          

 

EAS Academy offers a $1,500 scholarship to a current CPSDA student member who is studying to become a Sports Dietitian in a graduate academic or internship program.  This scholarship will be awarded to one individual in May 2013 who exemplifies all that it takes to be a successful Sports Dietitian. Scholarship criteria includes:

  • Leadership & Community Service
  • Nutrition-Related Experience
  • Paid or Unpaid Work Experience
  • Competitive Background in Sports
  • Sports Nutrition Mission Statement
  • Current Resume
  • Verification of Academic/internship Program
  • Three (3) References from Academic or Professional Sources

Completed nomination forms must be received by March 31, 2013. Follow this procedure to efficiently complete this application process:

  • Start with a blank word document to provide answers of up to 250 words each to the following four questions, then copy and paste your completed document into the box below labeled Historical Background:
  1. Please list your leadership and community service skills in 250 words or less
  2. Please describe any nutrition-related professional experience in 250 words or less
  3. Please list any work experience and dates of that employment in 250 words or less
  4. In a few concise sentences, write your sports nutrition mission statement, including your strengths and challenges as an aspiring Sports RD, as well as what you see as what your contributions to this field will be. Also include your vision for the future of Sports Nutrition.
  • Fill in the boxes below starting with name and concluding with Academic Major/Minor; double check that your word document has been properly copied and pasted into the Historical Background box, then click the submit button at the bottom of this page to send this information to Ms. Mandel. 
  • Finally, you will need to send an E-mail to Ms. Mandel (chmandel@umich.edu) containing the following:
  1. Required: Your Resume
  2. Required: Verification of Academic or Internship Program Experience
  3. Required: A letter of reference must be E-mailed directly from three (3) separate academic or professional sources to Ms.Mandel at chmandel@umich.edu, specifying that the letter is in reference to (your name) "in regards to the EAS Academy Graduate Scholarship Award."
